Readiness Surveys
Below are 3 separate surveys. Feel free to complete one or all of them and use the scoring instructions below each survey. Your scores will not necessarily provide conclusive results about you or your current psychological state. The Satisfaction with Life Scale (SLS) (Author: Ed Diener, University of Illinois, Urbana-Champaign, 1993)
DIRECTIONS: Below are five statements with which you may agree or disagree. Using the 1-7 scale below, indicate your agreement with each item by placing the appropriate number in the line preceding that item. Please be open and honest in your responding.
1 = Strongly Disagree 2 = Disagree 3 = Slightly Disagree 4 = Neither Agree or Disagree 5 = Slightly Agree 6 = Agree 7 = Strongly Agree
______1. In most ways my life is close to my ideal. ______2. The conditions of my life are excellent. ______3. I am satisfied with life. ______4. So far I have gotten the important things I want in life. ______5. If I could live my life over, I would change almost nothing.
SCORING: Add the numbers you wrote next to each item. If you scored 25 or less, you may consider involving yourself in psychotherapy to learn how you can be more satisfied with your life. If you scored 26 or higher, you seem to be doing pretty well.
Subjective Happiness Scale (SHS) (Author: Lyubomirsky, Sonja, & Lepper, Heidi. S., 1997).
DIRECTIONS: For each of the following statements and/or questions, please circle the point on the scale that you feel is most appropriate in describing you (the scale has been reduced from its original 7-pt format to a 5-pt format for simplicity).
1. In general, I consider myself: 1-not a very happy person, 2-a little happy, 3-somewhat happy, 4-pretty happy, 5-a very happy person
2. Compared to most of my peers, I consider myself: 1-less happy, 2-a little less happy, 3-somewhat happier, 4-happier, 5-much happier
3. Some people are generally very happy. They enjoy life regardless of what is going on, getting the most out of everything. To what extent does this characterization describe you? 1-not at all, 2-a little, 3-somewhat, 4-a lot, 5-a great deal
4. Some people are generally not very happy. Although they are not depressed, they never seem as happy as they might be. To what extent does this characterization describe you? 1-a great deal, 2-a lot, 3-somewhat, 4-a little, 5-not at all
SCORING: If you obtained a score of 12 or less, you may consider involving yourself in psychotherapy to explore your perceptions of happiness regarding yourself and others. If you scored 13 or higher, you seem to be doing pretty well.
Personal Growth Initiative Scale (PGIS) (Author: Christine Robitschek, Ph.D., 1999)
DIRECTIONS: Using the scale below, circle the number which best describes the extent to which you agree or disagree with each statement.
1 = Definitely disagree 2 = Mostly disagree 3 = Somewhat disagree 4 = Somewhat agree 5 = Mostly agree 6 = Definitely agree

SCORING: If you obtained a score of 27 or less, you may consider involving yourself in psychotherapy to explore the extent to which you take initiative in life. If you scored 28 or higher, you seem to be doing pretty well.
